Identify the problem
Blistering and flaking paint on wood
One main cause of this is moisture below the surface, caused by paint having been applied over an improperly prepared surface.
Painting over a surface with dust, oil, or some other substance can cause the bond below the surface to fail.
The natural expansion and contraction with temperature change is too much for a compromised bond and the paint cracks, flakes and blisters when the moisture heats up and pushes out.
How to fix it:
Scrape all loose paint to a tight edge.
Smooth out the tight paint by sanding.
Prime area with primer and allow for proper drying time.
Caulk, fill holes and paint with proper finish paint.
How to avoid
All bare wood should be primed before painting.
Painted areas should be properly maintained to prevent cracks from allowing moisture behind the surface.
Caulk and fill any gaps or holes in trim before painting.
Blistering and flaking paint on drywall, plaster, or masonry.

This also happens when the paint been applied over an un-primed solvent based paint, glue, or dust.
